| Complaint FormsWage ComplaintThe Division of Labor will assist in the recovery of earned compensation. If you wish to file a wage complaint with the Division, you must fully complete and submit to the Division a “Request for Mediation” form. The form includes instructions on the complaint process and the authority of the Division of Labor.
If your employer owes you wages or compensation, and it is within the first 60 days of your termination/separation you may also wish to complete an optional form entitled “Demand for Payment of Wages," which is provided as an administrative courtesy. Do not send the "Demand for Payment of Wages" form to the Division of Labor. If your former or current employer’s payments to you are with a dishonored check, you may wish to send the "Notice of Dishonored Instrument" form, which is provided as an administrative courtesy. Do not send the "Notice of Dishonored Instrument" to the Division of Labor. Immigration ComplaintThe Division of Labor will investigate employers who are not complying with the Employment Verification Law and contractors and subcontractors not complying with the Public Contracts for Services and Illegal Aliens Law. If you wish to make a complaint you must fully complete the form(s) below and submit to the Division.
